About Your Office Visit
Please prepare for this by bringing:
- Health Insurance Card/Information
- A friend or family member, if needed, to take notes/help with questions
- A list of all medications you are taking
- A list of your questions /concerns
- Copies of records from doctors related to your visit with us (if available).
- Completed FORMS below (optional, may be completed in office).
Patient Forms & Procedure Instructions
Dr. Shields performs procedures at varying locations depending upon your insurance. The majority of procedures are performed in the comfort of The Palo Alto Endoscopy Center. Patients with Medicare are performed primarily at Stanford Hospital and some at El Camino Hospital. If hospitalization is required he admits his patients at Stanford Hospital.

Insurance & Billing Policy
The offices of Dr. Shields is contracted with the following PPO Insurance companies. If you have an HMO, we will need a referral and authorization from your primary care provider. Our office is also contracted with VAH, but requires a referral and authorization by VAH. If you have any further billing or insurance questions please contact us directly.
